You will need a weigh slip if its in Ohio.
Trailers under 4K empty weight are not titled in Ohio. See the BMV site (http://ohiobmv.com/faq_vehicle_titling.stm#tog) and you can get a weight slip at any certified scale (Moving companies, sand & gravel yards, etc.) take a print of this PDF (http://publicsafety.ohio.gov/links/bmv5721.pdf)
It's easy

It should have come with a Statement of Origin. Just take that to the BMV and get your plates.

I bought my trailer new and just took the bill of sale (probably also consider the statement of origin?) to the BMV and got tags. The paper also had a weight listed on it that they went by.
